The VPExam Quality Improvement Project demonstrated how digital health tools, including the Eko DUO Digital Stethoscope with ECG, can transform transitional care for heart failure patients in skilled nursing facilities and home care. By enabling remote auscultation, arrhythmia detection, and structured data sharing, the program reduced 30-day hospital readmissions by 40% overall — and achieved 0% readmissions in the final months of the study. Nurses reported high satisfaction, and the integration of Eko technology into the VPExam telehealth platform supported timely medication adjustments, improved patient management, and expanded access to virtual cardiology care.
